THE VOCATIONAL REHABILITATION PROCESS FLOWCHART REFERRAL Write or call any office. Tell us you name, phone number, address, social security number and disability. We will let you know when the next orientation will be. We will contact you within 14 days ORIENTATION At the orientation, you will learn about VR services and what it takes to be eligible. You decide if you want to learn more. If you are, you will be given an appointment to see a counselor. INTAKE Meet your assigned counselor. Learn more about services and the process. If you are interested, sign an application for services. ASSESSMENT Tell your counselor about your work history and your work hopes. You and your counselor will review any problems you may have had getting, keeping and learning a job. Consider your strengths, resources, abilities, capabilities, concerns, interests and informed choice. Outcome: Determination of eligibility for services. Must be completed within 60 days of your application for services. PLANNING With your counselor’s help, you pick the kind of work you can do and want to do. Consider how much money you need to earn. Select services. Training? Perhaps college? Restoration? Need medical services? Rehabilitation technology? Job placement? Outcome: Your Individualized Plan for Employment. SERVICES Services will be provide by the providers selected and on the schedule agreed to. You and your counselor will monitor services to ensure that they are achieving the desired outcomes. Outcome: You achieve the employment goal noted on the plan. CLOSURE Your counselor will follow up with you for at least 90 days after you become employed. After that, if you are satisfied with your job and need no further services, your counselor will close your case. POST EMPLOYMENT SERVICES If something happens after your case was closed such that you need services to help you keep your job or find another job, call your counselor and explain what happened. We can help you again.
