Application Process
Family Child Care
1. Applicants must check with their landlords (if renting), town associations (if applicable) and insurance companies to first inquire if they would be able to conduct child care in their home.
2. Applicants must be in compliance with the zoning and building codes of their county (i.e. cannot have more than one complete kitchen, number of renters, etc.).
3. Applicants must complete the following:
a. A signed department application form (DHS 982);
b. Provide evidence of a satisfactory physical examination and TB clearance for all household members;
c. Two letters of reference;
d. Completed employment history clearance forms (DHS 959); and
e. Child abuse/neglect history check and criminal history check (DHS 948).
4. Upon receipt of the completed documents indicated above, 3a-3e, a licensing worker will conduct a home visit to inspect the home as well as to review the licensing rules with the applicant.
Group Child Care
1. Applicants must insure that the facility is in compliance with the building, zoning and fire codes for their county.
2. Applicants must complete the following:
a. A signed department application form (DHS 951, DHS 952, DHS 953, and DHS 954);
b. Provide evidence of a satisfactory physical examination and TB clearance for all staff;
c. Employment history clearance forms for all staff (DHS 959); and
d. Child abuse/neglect history check and criminal history check for all staff (DHS 948).
3. Upon receipt of the completed documents indicated above, 2a-2d, a licensing worker will conduct a site visit to inspect the facility as well as to review the licensing rules with the applicant.
